The Providers at Chestnut Ridge Family Medical office want to make sure you are familiar with your disease and encourage patient input in the decision-making process for treatment. There are a number of conditions that we treat, here are some more common conditions. Logs

You will see a list of printable or downloadable logs, at the bottom of screen, that you may use to print or download to your computer to keep track of your blood sugars or blood pressure at home. Bring this with you to your next office visit or send it to us via the patient portal for Dr. Dilamarter to review your at home readings.

WIC provides supplemental foods, health care referrals, and nutrition education for low-income pregnant, breastfeeding, and non-breastfeeding postpartum women, and to infants and children up to age five who are found to be at nutritional risk. http://www.fns.usda.gov/wic
